1Before you start
- Read all instructions before you begin.
- Keep your kit box (white and blue). You will need it to ship your sample back to LifeLabs.
- Drink plenty of fluids the day before collecting your sample.
- Check your kit contains all items listed. Place items on a flat surface preferably at or below waist level for collection.
- Use your kit before the “Return By” date on your request form.
- To ensure kit accuracy, return your kit as soon as possible after collection. Store in fridge until you can ship it.
Do NOT use this kit if you are:
- The kit appears opened, damaged or expired (see ‘Return By’ date on request form). If required, please contact LifeLabs at 1-877-849-3637 to replace the kit
- You are pregnant or within 2 months postpartum.
- You have Type 1 Diabetes.
Warning
- This kit contains sharp objects (lancets). Handle with care.
- Use with caution if you have a bleeding disorder or are on blood thinner medications.
- Should unexpected events not described in the instructions occur during collection, contact your nearest health care provider. For medical emergencies, contact your health care provider or 911. For kit failures or difficulties, contact LifeLabs at 1-877-849-3637.
2Collecting and shipping your sample

Record your sample collection date and time
Write the date and time of your collection on the pre-filled collection labels provided with this kit and then stick one on the sample tube vertically on top of the original tube label.
Your sample will be rejected if this step is not completed.
4Returning your sample to us
Mail your package or drop off it at your closest Canada Post

5Get your results
We will notify you when your results are ready. This normally takes few weeks from when your package is received at the lab.
