Online:

Add LifeLabs Medical Lab (ONT) as a payee and enter your 12 digit alpha numeric Customer number as the Account number.

 

Telephone:

Add LifeLabs Medical Lab (ONT) as a payee and enter your 12 digit alpha numeric Customer number as the Account number.

 

Teller:

Bring a copy of your invoice with you and make a payment using your 12 digit alpha numeric Customer number as the Account number.

 

 

Call the Accounts Receivable Department toll-free at 1-888-265-5227 to pay by Visa or Mastercard.

Credit Card

Complete your Visa or Mastercard information on the bottom section of the Invoice and mail to:

Cheque or Money Order

Prepare a cheque or money order by quoting the Invoice number and making it payable to “LifeLabs LP”. Please mail the invoice and your payment to:
