LifeLabs provides supplies and containers required for the collection of laboratory specimens referred to LifeLabs. To ensure supplies support the provincial protocols, please order your supplies from the province in which you are located.

To request supplies, complete either the Healthcare Provider Phlebotomy Order Form or Healthcare Provider (No Phlebotomy) Order Form and forward to your local LifeLabs location. Arrangements can also be made for delivery or pick-up by phoning Customer Care Centre: 1-877-849-3637.
