LifeLabs provides supplies and containers required for the collection of laboratory specimens referred to LifeLabs. To ensure supplies support the provincial protocols, please order your supplies from the province in which you are located.

Kits Order Forms

The Fecal Immunochemical Test (FIT) detects hidden blood in stool, an early sign of colorectal cancer. HCPs can order FIT kits, which include everything patients need to collect a stool sample at home and return it for lab analysis. Results go directly to the HCP. Available to all of Canada (excluding BC)

The HPV test screens for high-risk strains of the Human Papillomavirus, linked to cervical cancer. HCPs will soon be able to order HPV self-collection kits, allowing patients to collect samples at home for lab analysis. Results are sent directly to the HCP. (Coming soon).

The HbA1c test measures average blood sugar levels over the past 2-3 months, helping to monitor diabetes. HCPs will soon be able to order HbA1c self-collection kits, allowing patients to collect samples at home for lab testing. Results will be sent directly to the HCP. (Coming soon).

To request supplies, complete either the Healthcare Provider Phlebotomy Order Form or Healthcare Provider (No Phlebotomy) Order Form and forward to your local LifeLabs location. Arrangements can also be made for delivery or pick-up by phoning Customer Care Centre: 1-877-849-3637.
